Survived by her dad Michael Wade Sr, siblings Calvin Howard, Lori Wade, Michael Wade Jr, Christine Leith. And her many nieces and nephews, Calea, Kathy, Do, Sonny, Myra & Keith, Sammy, Dawn & Louie, Mary Crystal, Debowance, Dezzy & AJ, Shania, Dathen, Andrew, Denaya, Deslin, along with her many grand-nieces/nephews. Whom she loved very much.

Phyllis graduated from Intermountain High School 1983, and received her Associate of Applied Science and completed the Surgical Technician Program from High Tech Insitute in 2006. She was a surgical technician with plans of becoming a surgeon. Her plans changed when she was diagnosed with diabetes, which took a toll on her body. During her life, she held many jobs living most of her life in Minneapolis/St. Paul. It wasn’t until later in life she decided to move up north to be with family. Having no children herself, she treated every one of her nieces and nephews as her own. If Phyllis wasn’t at white oak, she would be home chillin’ with her nieces & great nieces/nephews. She was loved her job, and always had a good time with coworkers at White Oak.
